What activities does IMA offer at the wellness centers?

At our wellness centers, we offer social activities, health education classes, physical activities, arts & crafts, exercise equipment, salon services, and more.

How do I locate the nearest wellness center?

What are IMA’s hours of operation?

Most locations are open from 8:30am to 5:30pm Monday to Friday.

How do I schedule transportation for appointments with IMA?

Patients can contact their doctor's office prior to their appointment or schedule transportation at the time they request their next appointment.

I am a new patient. How can I get my previous medical records to IMA?

Fill in the Universal Authorization Form here and bring it to your next visit. We will take care of getting your medical records from your previous medical provider.

Can I get a flu shot at the doctor office?

Yes, patients can get flu shots at their doctor’s office during flu season.

How quick can I get my medical prescription?

Patients can get their medical prescriptions 24 to 48 hours after request.

How do I schedule my annual wellness visit?

Call your doctor’s clinic or 855-213-2329 to schedule your annual wellness visit.

If I incur a bill, how do I easily pay it?

How can I get my IMA medical records?
